CONFIDENTIALITY OF RECORDS:
Confidentiality assures that access to student records is provided to only those individuals or agencies with legitimate educational interests. Policies and procedures for confidentiality apply throughout the stages of collection, storage, disclosure and destruction of records, including electronic records. The State and City of New York require strict adherence to their mandates relating to the critical issue of confidentiality.
Every service provider must preserve the confidentiality of all information, electronic and/or hard-copy data, both historical and current data, shared, received, collected, or obtained as a result of their services. No disclosure, re-disclosure or release of such data or information is to be made, permitted, or encouraged. Please ensure that (a) all records containing personally identifiable information are maintained in secure locations; (b) any records that are carried with any individual who travels to a variety of locations to deliver services are kept secure and confidential when off-site; (c) you adhere to all legal requirements that protect early intervention records containing sensitive information (such as sexual or physical abuse, HIV status, treatment for mental illness, the Child’s parentage, etc).
The comprehensive Confidentiality Policy was provided to each service provider. Please review it to be sure that the specifics, as explained therein, are followed.
